Tdap Vaccine and Meningococcal Conjugate Vaccine for all 7th graders.
North Carolina state law requires all 7th-grade students to receive one dose of Tdap (tetanus, diphtheria, pertussis) and one dose of meningococcal conjugate vaccine. Your child will need to bring a copy of the shot record to school on the first day of 7th grade. After the 30th day of school, students who are not in compliance will not be allowed to attend school, per state law. Please contact your health care provider or the Mecklenburg County Health Department to schedule an appointment.


CMS requires that you now submit 2 proofs of Mecklenburg County residency when you are changing your address.

  • Complete the Change of Address form information (see below).

  • Address changes cannot be made unless 2 proofs of residency are provided at the time of the request.

  • All address changes must be completed in person.

  • Please be advised that an address change may change transportation for your child.


Southwest is a uniform school.  Please see below for the uniform policy.  


Please come into the school between the hours of 8-10:30 to withdraw your child.  You will have to filled out the withdrawal form,  return any books, clean out the student locker and pay any overdue fees.
